International traveler arrival form provisioned by CCMC
As provisioned by CCMC (COVID-19 Crisis Management Center) all international travelers arriving in Nepal must filled their Online arrival form. Form is to be filled up mandatory by all the ingress passengers. In the form arrival traveler are required to fill personal detail, family detail, travel history (in past 30 days), current resident country, quarantine details, and health information.
